Creating an Evaluation Template is the first step in assigning evaluations to your employees. Ensuring your template is set up correctly will help your evaluation process run smoothly and collect the information you need.
This article applies to HR Admins responsible for creating and managing evaluation templates.

Template Builder
Evaluation templates define the structure of your evaluations.
Create a New Template
- Click Evaluations from the left navigation menu.
- Select Templates.
- Click Create New Template.
- Configure the template settings.
Template Fields
Template Name
- Internal name visible only to HR users.
- Use clear naming if creating multiple templates.
Scoring Enabled
- Toggle on to assign point values to questions.
- Enables scoring for evaluations.
By default, employees will see their score as a percentage on finalized evaluations. To display labels instead, you must create Scoring Labels.
Display Scoring in Finalized Evaluation
- Controls which participants can see scoring (Employees, Managers, Peers).
- Toggle off to hide scoring from selected participants while still collecting scores.
Description
- Optional internal description.
- Visible only to HR users when editing the template.
Sections
Sections divide the evaluation into structured parts and determine who completes each section.
Add a Section
- Click + Section.
- Configure the section settings.
Section Fields
Section Title
- Visible to employees and managers during the evaluation.
Who should complete this section of the evaluation?
Select one:
- Employee only
- Manager only
- Both employee and manager
- Peers only
Include Section in Finalized Evaluation
- Toggle off to hide the section from the finalized evaluation.
Examples of hidden sections:
- Peer feedback for manager review
- Manager-only compensation recommendations
- Internal employee feedback about company culture
Section Description
- Optional instructions displayed during the evaluation.
Add Content
- Click + Section to add more sections.
- Click + Question within a section to add questions.
Questions
Questions collect responses from employees, managers, or peers.
Add a Question
- Click + Question within a section.
- Configure the question settings.
Question Fields
Question Title
- The prompt participants will answer.
Question Description
- Optional instructions shown during the evaluation.
Question Type
Choose one:
Multiple Choice
- Select from predefined answers.
- Can allow single or multiple selections.
Dropdown
- Select one answer from a dropdown list.
Text
- Open-ended response.
- Scoring is not available.
Label (Answer Options)
- Required for Multiple Choice and Dropdown questions.
- Example:
- Always
- Sometimes
- Never
Points
- Assign point values when scoring is enabled.
- Example:
- Always → 10
- Sometimes → 5
- Never → 0
Required
- Enabled by default.
- Toggle off to make the question optional.
Comments
Set as:
- Optional
- Required
- Disabled
Question Controls
Use the icons to:
- Copy a question
- Move a question up or down
- Delete a question
Summary
The Template Summary provides a real-time overview of your evaluation structure.
Section and Question Overview
- Displays all sections and questions.
- Click any item to jump directly to it for editing.
- Updates automatically when reordered.
Total Score Breakdown
- Displays total possible points when scoring is enabled.
Save Template
- Click Save Template to save your progress.
Save & Preview
- Click Save & Preview to view the evaluation as participants will see it.
Preview, Edit, and Archive Templates
All templates are managed from the Evaluations → Templates page.
Edit
- Click Edit to update sections, questions, or settings.
Preview
- Click Preview to view the evaluation layout.
Note: All sections will display in preview, including Employee-only or Manager-only sections.
Duplicate
- Click Duplicate to create a copy of an existing template.
Archive
- Click Archive to remove a template from active use.
- Archived templates can be restored if needed.
Important Notes and Limitations
- If scoring labels are not assigned, employees will see scores as a percentage.
- Sections hidden from finalized evaluations will still be visible to HR and managers before finalization or when viewing non-finalized evaluations.
- Text-type questions do not support scoring.
- Preview mode displays all sections, regardless of participant assignment.
- Archived templates can be restored but cannot be used unless reactivated.
